Emerson Elementary School will hold its annual kindergarten enrollment clinic on April 12 from 5:30 to 6PM. Children who will be 5 years old on or before August 1, 2018 and live in the Emerson School District or have school choiced to the district need to be registered at this time. The clinic will be held in the elementary library. Children will not need to attend this clinic.
Parents must have the following documents:
- ONE of the following: birth certificate (must have been issued by the Department of Vital Records of the state of child’s birth), a statement by the local registrar or a county recorder certifying the child’s date of birth, an attested baptismal certificate, a passport, an affidavit of the date and place of birth by the child’s parent or guardian, previous school records, or US military identification.
- Up-to-date immunization record
- Social Security Card
- Proof of physical examination done within two years of date of enrollment
- Medicaid Card if the child is covered by Medicaid
